His bundle pacing, selective and non-selective: are they equally safe and effective?

چکیده

Abstract Background Permanent His Bundle Pacing (HBP) is the most physiological form of ventricular pacing. bundle stimulation can be selective (s-HBP) or non-selective (ns-HBP). Few comparative data in terms safety and efficacy among two are available literature. Purpose Evaluate s-HBP ns-HBP identify predictors one other stimulation. Methods Prospective analysis patients with HBP implanted between December 2018 July 2021. The clinical instrumental parameters were collected at implant long-term follow-up. Follow-up both outpatient visits by remote monitoring systems. Results 130 need for antibradyarrhythmia therapy 26 an indication cardiac resynchronization enrolled, 134 (86%) had successful (34% 66% ns-HBP). There no significant differences populations exception presence right branch block (RBBB: 17.4% 34.1% ns-HBP; p = <0.05) baseline QRS duration (116.5±27.5 ms 129.9±34.7 <0.05). (Figure 1). At implantation average 16-month follow-up there electrical modalities. Twenty-one (15.7% population, 24% 12.5% p=0.38) conduction system disease progression, manifested either a increase pacing threshold (13.3% 10.2% recipients; p=0.64; Figure 2A) loss capture (6.5% 2.2% p=0.69). No statically progression found 2B). While seventeen who elevation underwent device output reprogramming, four patients, lost capture, single experiencing lead dislodgment (nS-HBP patient) required repositioning (8.7% 4.5% p=0.33). was significantly more vulnerable to atrial oversensing that sensitivity reprogramming (17.4% p<0.05). endpoints (cardiovascular death, heart failure, fibrillation, syncope) observed Conclusions In indicated stimulation, potential benefit represented burdened non-negligible number complications. Though detected medium-long term appears affected tissue disease, which resulted almost 2-fold (although statistically not significant) incidence repeated surgery. Autopsy analysis of the implantation site of a permanent selective direct his bundle pacing lead.

To date, there has not been direct visualization of the anatomic location of direct His bundle pacing (DHBP) leads in the human heart. The absence of such data has contributed to disagreement about the location of DHBP leads with respect to the plane of the tricuspid valve.1,2 We present an autopsy study of a patient who had previously had a DHBP lead implanted, showing unequivocally that the l...

His Bundle Pacing: Techniques and Outcomes.

Traditional right ventricular pacing prolongs the electrical and mechanical activation of the heart, which reduces the pump function, and in selected patients may lead to pacing-induced heart failure. Pacing the His bundle preserves the normal or near-normal activation pattern of the ventricles that prevents the detrimental effects of right ventricular pacing. This review discusses the techniqu...

assessment of deep word knowledge in elementary and advanced iranian efl learners: a comparison of selective and productive wat tasks

testing plays a vital role in any language teaching program. it allows teachers and stakeholders, including program administrators, parents, admissions officers and prospective employers to be assured that the learners are progressing according to an accepted standard (douglas, 2010). the problems currently facing language testers have both practical and theoretical implications but the first i...

The potential and reality of permanent his bundle pacing.

Right ventricular apex pacing can have deleterious effects. Our aims were to investigate how many patients referred for permanent pacing were suitable candidates for permanent His bundle pacing, and to determine the proportion in whom such pacing was successful. All cases of suprahisian block and most cases of infrahisian block (71.4%) were corrected by temporary His bundle pacing. However, per...

Long-term His-bundle pacing and cardiac function.

The article by Deshmukh et al 1 published elsewhere in the journal represents a tour de force for a number of reasons. The ability to chronically and selectively pace the His bundle not only represents an important methodological advance but also allows us to test the hypothesis that septal pacing in a fashion nearly identical to normal activation of the His-Purkinje system is superior to ventr...
